New Patient Forms

Please complete the appropriate New Patient form before your initial appointment.

Download the form and you can either;

print it out and bring it on the day of your appointment

or, fill it out on your phone/computer and email it to ballathiechiropractic@outlook.com.au.

If you can't do this, don't worry. We will have hard copies available in the office for you to use. For this option, please arrive 5 minutes earlier to allow time to complete the form.

Important Notes

Appointment Reminders

Please note that all appointment reminders will be via email.

Ensure that you record your appointment times or check your emails for your reminders.

Private Health Insurance

We are currently not set up to claim your private health insurance on-site.

However, we can provide you with the receipt and all the information so you may claim it yourself.

Cancellations

Please ensure a minimum of 24 hours notice if you are going to cancel your appointment.

If no notice is given, 50% of the appointment fee will be charged.
